Summary: xmlproc doesn't call startPrefixNS

Initial Comment:
when feature_namespaces is enabled, a sax parser should
call startPrefixMapping and endPrefixMapping each time
a new prefix/namespace is introduced in the document.

xmlproc parser doesn't call those callbacks at all .
