Chris,

I have pushed code to my github fork which emit WSDL for method faults
in all three places (types, portType operations, binding operations),
and to fix the actual soap:BODY XML emission for application-defined
faults as well.

It looked to me as though you still weren't going to be notified
directly via the pull request, so I am CC'ing here.
