Is the users address EXACTLY the same as the address in the accept
filter? Note that user at mail.example.com is not the same as
user at example.com.

Are you validating on envelope sender (USE_ENVELOPE_SENDER = Yes in
mm_cfg.py)? If so, is this users envelope sender EXACTLY the same as
the address in the accept filter?

If necessary, have the user send you a message in exactly the same way
as (s)he sends to the list and view the message source. Some MTAs will
create a Return-Path: header with the envelope sender address.
