i don't have time to comment in detail, and just skimmed your
message.  i'll respond again after ipc9, but just to answer one
question:

mailman preserves the case of email address, but it does case
insensitive comparison of addrs as keys.  the policy is that addrs
that differ only by case are considered the same user -- /but/ it
needs the case preserved addr to use as the recipient of the message.
that's because some mta -- believe it or not -- do care about the case
of the recip addr.

that may or may not make sense -- i'm still going to sleep on east
coast time, but waking up on west coast time.  not conducive to lucid
thought no matter how many gallons of coffee is consumed. :)
