My own view (but IANAL) is that it completely suffices to 
list yourself as author, date it, and state that the code is 
under the MIT license: e.g.,
:author: Charles R Harris <charlesr.harris gmail.com>
:date: 2006-04-25
:license: MIT http://www.opensource.org/licenses/mit-license.html
