In July of 98, in response to a problem Christian was having, 
Sam produced a patched calldll that used TLS storage so that 
a callback would happen on the thread that started the trouble.

I have no idea if that patch was incorporated in the 
mainstream, or what's happened to it. However, Christian is a 
well-known packrat ;-).
