Summary: debugger ``condition`` and ``ignore`` exception handling
Initial Comment:
Fixed ``ignore`` and ``condition`` functions in the
debugger (pdb.py) where they were not handling an
``IndexError`` exception which occurs when an invalid
breakpoint number is passed in.
Added a function ``do_show()`` which lists all
breakpoints. ``do_show()`` takes a filename pattern as
an argument.

I'll apply the fixes to do_ignore() and do_condition(), but I'm not so
sure about adding do_show(). This meaning of "show" would diverge wildly
from gdb's command of the same name.
