You may also want to look at:
   http://moinmoin.wikiwikiweb.de/ThemeMarket/FixedLeft
The download pointer is in ThemeMarket.

If you want even less clutter, the page trail can be turned off in
UserPreferences.  After that you have to modify the theme code or CSS.
