Summary: sgmlop and processing instructions

Initial Comment:
This patch adds correct handling of processing
instructions to sgmlop.

In XML mode a processing instruction will be terminated
at '?>' and not at '>'. (see
http://www.w3.org/TR/2000/REC-xml-20001006#sec-pi)
