Have you checked to see if Outlook has disabled the plugin?  Help-

(I'm not familiar with the "runtime error" message that was in the  
attachment you included.  It might be best to uninstall the source  
version (addin.py --unregister) and reinstall 1.0.4 if it is disabled).
