That line is normal. It means that the addin-in is closing (because you
closed Outlook). It will be at the end of every log where Outlook closes
normally.

And this is definitely not Outlook disabling the plug-in? i.e. you've
checked Help->About Microsoft Outlook->Disabled Items, and the
Tools->Options->Other->Advanced Options->COM Plug-ins list?
If there is no log, then Outlook is not starting the plug-in, so it has
become unregistered somehow, AFAICT.
