After several installs, one uninstall reboot, install...
Outlook Tools Options Other AdvancedOptions Com 
Addins -  Initially had MapPoint.  This was removed.
Add fails to indicate after browsing to 
spambayes_addin.dll

Can you try running "regsvr32 spambayes_addin.dll" from the
directory with the DLL, and send me the log from that?
