[Shtoom] bug report: OSX / audio / Python 2.3.4

This was user error on my part.  Or if it was a bug in Shtoom, then the  
bug was that shtoom/setup.py build_ext doesn't automatically invoke  
CocoaAudio/python_callbacks/setup.py build_ext.
