The second issue I was seeing has now been fixed. It was a problem 
in the code that removed an entity from the list of children of a given
entity. This bug has been lying there since the code was changed to
permit equivalent entities to reside in a layer, and I suppose it is
just luck that it hasn't appeared sooner. The fix has been sent to the
repo and is available for those using subversion by 'svn update'.
