quoting the README:

    copy the "Setup.in" file to "Setup" and edit it
    according to the instructions in the file.

quoting the Setup.in file:

    You can comment out the Tcl/Tk stuff if you
    don't need ImageTk, or if you're using a custom
    _tkinter instead (like in earlier versions of PIL).
