[Patches] Automatic patch notification messages

Exactly once per patch: when the patch is first entered in the database.

Status changes, as far as I can tell, go to the person who submitted
the patch and to the person who is the assignee.  There's probably a
bug that when the assignee changes, the new assignee gets mail but the
old assignee doesn't.
