No, it says
  BÍC:  knees or feet?

_ is a substitute for space in "q" encoding.

See section 4.2, no. (2), in RFC 2047.

It's used, because spaces are frequent in encoded headers and unencoded
spaces aren't allowed in an encoded-word.
