explains how you can copy the template(s) for the language(s)
you are interested into the list directory and modify them to
suit.  You should be able to add your <form>...</form> block to
archtoc.html (and you may want it in archidxhead.html as well).

(You can regenerate your archives with 'bin/arch --wipe mylist'.)
