I haven't heard of this problem from anyone else. My initial guess is 
there was some confusion over nomail vs. unsubscribe. They thought 
they unsubscribed, but in fact really turned on nomail instead, a 
very different beast. I wonder if there might not have been either a 
subdomaining issue or some unrecognized case sensitivity problem in 
looking up addresses. But that's just guessing.
