[Mailman-Users] Subscription confirmation problem

Just setting up Mailman for the first time, running on RedHat 6.2, Mailman
2.0beta5, Sendmail 8.9.3, Apache 1.3.12.

When subscribing to a list, request for confirmation comes through OK, after
confirming, message received back as held with "Message has implicit
destination" reason. List admin tries to approve subscription but there are
no options to Refuse or Subscribe the user. When checking the approve box on
the Administrative requests page the subscription request message is
released and gets posted to the list! The approval seems to be waiting in
the pending_subscriptions database and there does not appear to be any way
to release it. The list setting is for confirm only not for approval. User
subscription via the web page and via email produces the same results, list
admin subscribing the user works fine. Once subscribed everything else
appears to be working OK. It also appears that other admin type messages go
to the list!

Any help gratefully received as I am keen to get it running.
