It looks like upgrade does not set the per-user moderation bits
for every user for a previously-moderated list, yes.  It could
be argued that this is an UpdateOldVars() bug.

I suspect 2) below is just another instance of "incredibly snotty
email command parsing (honestly, it will complain about whitespace
I think..)

If the message was approved, then the Approved: header worked.
